Truvada & Generic PrEP (TDF-FTC): The Complete Guide
Truvada — a combination of tenofovir disoproxil fumarate and emtricitabine (TDF-FTC) — is the original PrEP pill, approved for prevention in 2012, and now available as a generic that costs less than a takeout lunch per month. It remains the most-prescribed, most-studied PrEP option in the world.

Brand vs. generic: any real difference?
No clinically meaningful one. Generic TDF-FTC must demonstrate bioequivalence to Truvada to be approved — same active ingredients, same amounts, same absorption profile. The inactive ingredients (fillers, coatings) can differ, which very occasionally matters for a specific sensitivity, but effectiveness is identical. Given the price difference, generics are the default choice for most new starts, and insurers increasingly require them.
Effectiveness and the adherence curve
TDF-FTC's protection tracks adherence closely. Modeling from the pivotal trials suggests roughly 96% protection at four doses per week and ~99% at seven for anal sex exposure; vaginal exposure requires stricter daily adherence because drug levels in vaginal tissue build lower and slower. The practical takeaway: aim for every day, and don't panic over one missed dose — but two or more missed doses a week starts eroding real protection, especially for vaginal sex.
TDF-FTC is also the only oral option with trial evidence behind on-demand 2-1-1 dosing for men who have sex with men.
Side effects and monitoring
Most people feel nothing. The most common early effect is mild nausea or headache in the first weeks ("start-up syndrome") that resolves on its own. The two things your prescriber actually monitors: kidney function (TDF can stress kidneys in susceptible people — a simple creatinine blood test at baseline and periodically) and bone density (small, largely reversible decreases observed in trials; relevant mainly if you have existing bone disease). Who might choose something else
Consider Descovy if you have kidney disease or osteoporosis risk; consider Apretude if daily pills don't fit your life. For everyone else, generic TDF-FTC's combination of track record, flexibility, and price keeps it the workhorse of HIV prevention.
