Descovy for PrEP: Who It's For (and Who It Isn't)
Descovy (tenofovir alafenamide + emtricitabine, TAF-FTC) is the second-generation PrEP pill. It delivers tenofovir more efficiently to the cells that matter, using a much lower dose in the bloodstream — which is easier on kidneys and bones. But it carries one major limitation people are often surprised by.
The short version
- Daily pill, same ~99% effectiveness for the populations it's approved for.
- Gentler on kidneys and bone density than Truvada/TDF-FTC.
- Not approved for people at risk from receptive vaginal sex — the trials didn't include them.

The vaginal-sex limitation, explained
Descovy's approval trial (DISCOVER) enrolled cisgender men who have sex with men and transgender women. It did not enroll cisgender women, so the FDA approval excludes HIV risk from receptive vaginal sex — not because Descovy is known to fail there, but because it hasn't been proven there and tenofovir behaves differently in vaginal tissue. For most people with healthy kidneys, the honest answer is that generic TDF-FTC is equally protective at a fraction of the system-wide cost — which is why many insurers require trying it first. Descovy earns its place for people with reduced kidney function (eGFR concerns), a history of kidney injury, osteoporosis or significant fracture risk, or who developed side effects on TDF.
Getting Descovy covered
Because of the price gap, some plans apply prior authorization or "step therapy" (try generic first). If you have a clinical reason for Descovy, your prescriber documents it and approval is routine.
