DoxyPEP Dosage: Exactly How and When to Take It
DoxyPEP dosing is refreshingly simple, but the details — timing windows, multiple encounters, what you can't take it with — are where questions pile up. Here's the complete picture as used in the clinical trials and CDC guidance.
The regimen
- 200 mg of doxycycline, taken once, as soon as possible after sex — ideally within 24 hours, no later than 72 hours.
- Maximum 200 mg per 24 hours, no matter how many encounters.
- Take with a full glass of water and don't lie down for 30 minutes after (doxycycline can irritate the esophagus).
- Taking it with food is fine and reduces nausea.
Common timing scenarios
| Scenario | What to do |
|---|---|
| Sex Friday night, remembered Saturday morning | Take 200 mg now — well within the window. |
| Sex Friday night and Saturday night | 200 mg Saturday morning covers Friday; another 200 mg Sunday (24h after the first dose) covers Saturday. |
| Multiple encounters in one night | One 200 mg dose covers them all. |
| Remembered 4 days later | Outside the window — skip it, and get tested in 2–4 weeks instead. |
| Ongoing daily sex (e.g., a vacation) | Effectively 200 mg daily; fine short-term, but discuss patterns like this with your prescriber. |
What interferes with doxycycline
Doxycycline binds to polyvalent cations — calcium, magnesium, aluminum, iron, zinc. Practically: separate your dose by at least 2 hours from antacids (Tums, Maalox), dairy-heavy meals, and mineral supplements (multivitamins, iron, magnesium, zinc). Taking them together can meaningfully reduce absorption. Also flag to your prescriber if you take isotretinoin (never combine), warfarin, or certain seizure medications (carbamazepine, phenytoin) which speed doxycycline's clearance.
Does timing really matter?
Yes — the mechanism is stopping bacteria before they establish infection, so earlier is better. The trials used 72 hours as the cutoff; think of 24 hours as the target and 72 as the deadline. Many users simply take the dose when they get home. That habit — dose on the nightstand, taken before sleep — is probably the single best adherence trick.
What DoxyPEP dosing is not
It is not daily doxycycline (that's a different strategy with a different risk-benefit profile, not currently recommended for STI prevention), it is not a treatment for an active STI (if you have symptoms, you need testing and full treatment courses), and it does not prevent HIV — that's PrEP's job. Questions about your specific pattern of use are exactly what a telehealth consult is for.
